Appointment booking is provided through ChiroSpring. Information submitted through that booking service is handled according to its own policies.

To request medical records, patients must submit a written request to the office for their medical records. Contact information must be provided on the medical record request such as name, phone number, and address of person requesting medical records. Medical records will be made available for pick up or mailed out via USPS certified mail not later than the 15th business day after the date the health care provider receives a written request from a person for their personal medical record. Patient must provide the requested record format in the written request.
If you are requesting medical records on someone behalf, then a medical record release form must be filled out and signed by patient to release medical records to someone other than patient. To file a complaint against a medical provider at this establishment, please submit all complaints to the Texas Board of Chiropractic Examiners.
